Commercial Plans
Employer-sponsored and individual commercial insurance plans
We are in-network with UHC commercial plans (HMO and PPO/open access) and most UHC Medicare Advantage plans, including Group Medicare Advantage (PPO). We are out-of-network with UHC Marketplace / Individual Exchange plans, UHC Community Plan / Medicaid, UHC Core, Navigate, or Charter plans, UHC Medicare Advantage Private Fee-For-Service or Medical Savings Account plans, and Medicare Select plans. If you have a UHC plan, please call our office to confirm whether your specific plan is in-network.
